The Schuster Clinic is accepting new patients. All new patients must have a referral from their primary or specialty physician. Once your physician has referred you to see an endocrinologist, please call our office to schedule an appointment
Thank you for scheduling an appointment with Dr. Schuster. We look forward to seeing you. For registration purposes, please arrive approximately 15 minutes prior to your scheduled appointment. We ask that you fast at least 8 hours prior to your appointment (you may have water and any medications you would normally take). Please avoid placing any perfume on your neck on the days of your appointments with Dr. Schuster. You should plan to be at our office for approximately two hours for your initial consultation, which includes a physical examination and laboratory studies.
We require your medical records be present in our office PRIOR to your initial visit. Dr. Schuster will review your medical history before your appointment. Please have your doctor’s office send us any recent laboratory results as well as any ultrasounds or xrays with the reports, prior to your visit. You may also have records faxed to our office at 952-920-8127.
Please bring the following with you on the day of your appointment:
- New Patient Questionnaire Form
- Insurance Card(s)
- Driver’s License or ID card
